When a first time requester asks for copies of all documents, send copies of all separation documents; also send copies of personnel and medical documents which show the following information:

AND, if character of service is Other Than Honorable (OTH), send the Facts and Circumstances pertaining to discharge.
* Send only those found in the record. It is not necessary to have the ORU pull the clinical record.
Some of the information you must provide will be found on the following service record documents:
| Army: | DA Forms 20, 2-1, 66, or 24 |
| Air Force: | AF Form 7, 11, or 1712 |
| Navy enlisted: | NAVPERS 601-14 (or 601-15 or 1070/601-14 or 15) and service record book pages 4 thru 6, 8, 9, 12, 13 |
| Navy officers: | Fitness Reports, yellow cards, all separation documents, and any pages that show any of the following: appointment as an officer; award citations; retirement or discharge. If there are no yellow cards, send any page(s) showing change of rank and any "Statement of Service" page(s) showing active duty and inactive duty. |
| USMC: | NAVMC 11 and service record book pages 3, 5, 6, 8, 9, 12, 13, 17 |
| USCG: | CG 3312A and service record book pages CG 3303, 05, 06, 07 |
IF the record is on microfiche, send paper copies of pertinent pages to the requester unless a microfiche is specifically requested.
IF the above information has been provided, and a second request is received indicating that all records are needed, send a copy of all personnel and medical records (Keeping in mind the normal restrictions that apply to releasing personnel and/or medical information).
